SECTION 8 — PAY & INDEPENDENT CONTRACTOR ACKNOWLEDGMENT

_____ I understand that I am responsible for my own taxes, insurance, reporting, and business expenses unless otherwise agreed in writing.

_____ I understand that pay may vary by job type, complexity, approval status, and company policy.

_____ I understand that pay is issued according to the company’s approved payment schedule and method.

_____ I understand that deductions, chargebacks, or reduced dispatch priority may occur for negligence, damages, incomplete work, customer complaints, callbacks, or policy violations, subject to applicable law and company review.

SECTION 9 — COMPANY POLICY ACKNOWLEDGMENT
